The National Disability Insurance Scheme (NDIS)
The National Disability Insurance Scheme (NDIS) – www.ndis.gov.au – includes information such as “What help can I get?”, “Access requirements” and a wide range of additional information.
Anyone with a disability or who is affected by disability is able to obtain information about the planning process, supports, managing your plan and supports and self-managing funds in your plan, etc. Fact sheets, publications and resources are provided on the website along with a “My Access Checker” to see if you are able to access assistance from the National Disability Insurance Scheme.
Once you have funding (or self-funded individuals), please contact us to set up a personalised consultation in the convenience of your own home or at our office in Gawler. The consultation will cover areas such as your specific situation, the funding available, and your requirements for assistance including times and support staff. We will take the time to explain things and answer any questions you may have.

Health Care Plan
Once we understand your needs and the administration details are complete, we will then prepare a Health Care Plan in consultation with you. This confidential Health Care Plan will be used to assist staff with the provision of high quality care. All the details of the Health Care Plan are looked after by our competent office staff; you don’t need to worry about filling in more forms.
The office staff will organise all other necessary aspects for your service to commence. Under certain circumstances, a “meet and greet” can be arranged for you to meet your team of Support Staff prior to services commencing. Rostering of your support team will be completed by the office.

With a clear understanding of your needs and all the finer details taken care of, we are now ready to commence your service. The Client Service Team will be in touch soon after services commence for your valuable feedback.
